General Instructions

This application form is to be filled out in its entirety to prevent delays in processing.  It is to be used to apply for tuition  for our internal courses or to prepare for our Authorized College Board’s Advanced Placement ® (AP) course exams.  Applicants are required/strongly urged to read the Online code of conduct, Course Fees and Academic. Not all courses are offered each year.  This depends on the number of applicants and availability of staff. Please read the Registration Requirements (in the Applications web page) prior to completing this application form.

Please read through carefully before filling in.

Subject Selection
If you have answered in the affirmative to to all the questions concerining the basic subjects, then, you are attesting that you have met the minimum requirements to register for any of the basic AP or our Internal subjects taken in Year 1.  Year 2 courses should only be taken if you feel that you have adequately mastered the relevant year 1 course or their equivalent.
Advanced and Year 2 and Year 3 Courses
If you wish to register for more advanced AP courses such as Calculus BC, Calculus-based  AP Physics C or any other combination of courses, please write in the AP courses  or their equivalents previously taken and the respective grades obtained in  so that we can better assess your suitability for the choices that you will make below..

Registration and  Payments
SMM Academy will respond to your application via email and send you the payment information within 5 days of application.  If accepted, You you will pay for the Course Certificate(s) online through PayPal or Square when invoiced.
